Vaccination Event Documents

Put both of these documents at every vaccination and registration station. When someone has a question, the answer should be within reach of their hand.

Using sheet protectors increases durability and reusability and allows for cleaning. Sheet protectors also allow printing everything one-sided then putting a front and back of a form into the protector to make a two-sided form. Saves having to print one side of the forms then flip them over and feed them back through the printer for the other side.

5/24/22 update

This update includes changes to both the VCS and VRT documents. Both the FDA and CDC issued updated guidance for a Pfizer-BioNTech booster 5+ months after the last primary shot for ages 5-11 years old. In addition, CDC is strengthening its recommendation that those 12 and older who are immunocompromised and those 50 and older should receive a second booster dose at least 4 months after their first.

4/7/22 update

The CDC issued slightly updated guidance for a 2nd booster 4 months after the 1st booster of any vaccine for people of age 50+ years old who received Janssen as their primary dose. These individuals have no health requirements for the 2nd booster.

3/29/22 update

This update includes changes to both the VCS and VRT documents. Both the FDA and CDC issued updated guidance for a 2nd mRNA-only booster 4 months after the 1st booster of any vaccine for immunocompromised individuals 12+ years old and people of age 50+ years old. Additionally, the CDC authorized a 2nd mRNA-only booster 4 months after the Janssen booster taken by individuals who also received Janssen as their primary dose. These individuals have no health requirements for the 2nd booster.

2/24/22 update

This update includes changes to both the VCS and VRT documents. The VTR changes on the front page will be obvious. An extra vial line has been added and the "2nd shot" interval has been updated. The VCS has been updated for new CDC guidance regarding the interval between the first and second shots of the primary mRNA series. Both the Moderna and Pfizer-BioNTech interval for the second shot of the initial series has been extended to as long as 8 weeks for certain groups.

This FDA update reduced the Moderna booster wait time to 5 months after the second shot of the initial series, down from 6 months.

1/6/22 update

This update is to add the Pfizer-BioNTech Gray top vial drawing instructions to the VCS, and to provide a better description of the 3rd shot for 5-11 year olds. No substantive changes to the VRT, just some reformatting. The link for this web page has been added to both documents.

1/3/22 update

This update is in response to FDA/CDC guideline changes for Pfizer-BioNTech vaccines only. Both the VCS and VTR have been updated.

Boosters now authorized for 12+ years old

Booster wait time decreased to 5 months from 6 months

Immunocompromised children ages 5-11 are now authorized for a 3rd Catch-up shot

Although the CDC recently changed their COVID isolation and quarantine periods, they did not update their instructions regarding how long to wait for vaccination after COVID infection or exposure. They say they don't have enough data to establish firm guidance for a particular waiting period after COVID infection. One study says to wait at least 2 weeks after an infection to give a COVID vaccine in order to let the body's immune response fully develop against the infection first. It's an open question since level of immunity cannot be tested for the public.

12/12/21 update

This update is not in response to CDC guideline changes. It is to add existing CDC booster guidelines for people who got their initial vaccine series in another country. You can see the new Pfizer Booster International Rules line on the VCS thumbnail below. The explanation for who is qualified for this Pfizer booster is on the back of the VCS. To make room for it the information box about how soon someone can get vaccinated after COVID-19 has moved to the back of the VRT form. Also added to the back of the VRT is an explanation of how to use the age brackets to determine vaccine eligibility without doing any date calculations.

If you're not using the VRT, and want to keep the "vaccinated after COVID-19" information on the back of the VCS instead of having to use the VRT just for that, use a sheet protector. Put the front side of this VCS, and the back side of the 12/9/21 VCS, facing out. That gives you the best of both worlds in a "single" document.

The other big change with the update is the cosmetic removal of the Community Nursing Initiatives branding. This allows a little more space for information and consolidates the two previous versions of the forms I was making available. This will be the format going forward.

12/9/21 update

This update is in response to CDC guideline changes. It adds new CDC booster guidelines for 16-17 year-olds.

There is a new document, the Vaccine Registration Table form. It is intended to hold the pieces of information which change on a daily basis. In the Age-Based Dosing section, if you lay a birthday (ID, vaccination card, registration form) beside the brackets you can immediately see which vaccines the client is eligible for without doing any date calculations.
